Case managers assist patients and families in evaluating, arranging, and coordinating available support systems needed now or in the future. Depending on expertise, they can assist in determining next steps in legal situations, healthcare, benefits, and more. The mission of NewBridge is to provide older adults a bridge to successful aging. Case Management provides older adults support needed to live independently by identifying resources and coordinating services. Information, referrals, and advocacy provided through case management home visit assessments of each older adult’s individual needs.
COALITIONS
Coalitions are a great source of information and a good place to start researching specific areas of interest (for example, senior housing options). Case managers are oftentimes on staff to help coordinate services and assistance when needed.
